Cauda Equina syndrome

Cauda Equina syndrome is a serious medical emergency that requires urgent medical attention. If treatment is delayed, this can result in catastrophic injuries.

What is Cauda Equina?

The Cauda Equina is a group of nerves at the bottom of the spinal cord that look like a horse’s tail. Cauda Equina Syndrome is the compression and swelling of those nerves. Damage to the cauda equina can result in permanent loss of bladder and bowel control, poor mobility and, in severe cases, paralysis.

Causes of Cauda Equina?

  • Traumatic injury
  • Disc herniation (bulging)
  • Spinal stenosis (narrowing)
  • Spinal tumors
  • Inflammatory conditions
  • Spinal infection
  • Clinical negligence

Symptoms of Cauda Equina?

  • Severe low back pain
  • Pain that radiates down the back of the leg from the buttocks
  • Weakness and/or numbness in one or both legs
  • Numbness in the groin, buttocks or inner thighs also known as the ‘saddle’ area
  • Difficulty passing urine and/or incontinence
  • Decreased anal tone and dribbling incontinence
  • Sexual dysfunction

Treatment

If there is any sign at all of a Cauda Equina palsy or a failure to improve with conservative management, then urgent decompression surgery is required to relieve the pressure on the nerves to prevent permanent damage occurring.

Most experts are agreed that timing of surgical intervention does influence the eventual outcome. The window of opportunity is very narrow as surgery should occur within 48hours of the onset of symptoms. However, even with prompt treatment you may not make a full recovery as some damage may well have already occurred, but delayed treatment could result in further damage that would otherwise have been avoided.
