Medication and Prescription Errors

In hospital, it is the nurse who usually administers the medication. Prior to administration of medication it is the Registered Nurses responsibility to check the patients name and date of birth against the patient’s identification band and then against the medication chart.

When administrating medication the nurse is duty bound to abide by the Nursing Midwifery Councils Guidelines for administration of medicines and must ensure that the right medication is given to the right patient at the right time in the right dosage and also by the right route.

Medication errors commonly involve children who are frequently given the wrong dosage as the medication hasn’t been calculated correctly against their age or weight, and the very sick who require urgent care.

Although many medication errors cause no harm, the administration of the wrong medication or the wrong dose of medication can have serious complications. For example, the wrong dose of insulin may result in serious harm to the patient, and even death.
